  2. Your Rights Regarding Your PHI

    You have the following rights under HIPAA:

    Right to Access and Obtain Copies

    You may inspect or request a copy of your medical records and billing information, subject to limited exceptions.

    Right to Request Amendments

    You may request corrections to your PHI if you believe it is inaccurate or incomplete.

    Right to Request Restrictions

    You may request limitations on how we use or disclose your PHI. While we are not required to agree to all requests, we will comply when required by law.

    Right to Confidential Communications

    You may request that we contact you in a specific way or at a specific location.

    Right to an Accounting of Disclosures

    You may request a list of certain disclosures of your PHI made outside of treatment, payment, and health care operations.

    Right to a Paper Copy of This Notice

    You may request a paper copy of this Notice at any time, even if you have agreed to receive it electronically.

    Right to Be Notified of a Breach

    You have the right to be notified if a breach of your unsecured PHI occurs.

  3. Client Responsibilities

    As a client of Primal Point Health, you are responsible for:

    • Providing complete and accurate health information
    • Participating in your care plan and informing providers of changes
    • Treating staff and property with respect
    • Keeping appointments or providing a timely cancellation notice
    • Fulfilling financial responsibilities related to services received
  4. Our Duties

    Primal Point Health is required by law to:

    • Maintain the privacy and security of your PHI
    • Provide you with this Notice of our legal duties and privacy practices
    • Abide by the terms of the Notice currently in effect
    • Notify you following a breach of unsecured PHI as required by law
    • Comply with applicable federal and state privacy regulations
  5. Complaints

    If you believe your privacy rights have been violated, you may file a complaint without fear of retaliation.

  6. Changes to This Notice

    We reserve the right to change this Notice and make the revised Notice effective for all PHI we maintain. Any updates will be posted on our website with a revised effective date. If significant changes are made, additional notice will be provided as required by law.
