At Primal Point Health, we are committed to providing high-quality, compassionate, and ethical care. As a client receiving services, you have specific rights and responsibilities as outlined by the State of Maryland and the Accreditation Commission for Health Care (ACHC).

Client Rights

As a client of Primal Point Health, you have the right to:

1. Respect and Dignity

  • Be treated with respect, consideration, and dignity at all times.
  • Receive care in a safe environment free from abuse, neglect, exploitation, or harassment.

2. Privacy and Confidentiality

  • Have your personal health information kept private and confidential in accordance with HIPAA and applicable laws.
  • Review, obtain copies of, and request amendments to your medical records as allowed by law.

3. Information

  • Receive clear, accurate, and understandable information about your diagnosis, treatment options, risks, and benefits.
  • Know the identity, credentials, and professional status of individuals involved in your care.

4. Participation in Treatment Decisions

  • Actively participate in decisions regarding your treatment plan.
  • Refuse treatment and receive information about potential medical consequences of refusal.

5. Informed Consent

  • Receive sufficient information to provide informed consent prior to the start of treatment or services.
  • Be informed about advance directives and have them respected in accordance with the law.

6. Access to Care

  • Receive services without discrimination based on race, color, religion, sex, sexual orientation, gender identity, disability, age, or national origin.
  • Receive timely access to care and prompt responses to service requests.

7. Complaints and Grievances

  • Voice concerns or complaints regarding care without fear of retaliation or denial of services.
  • Receive a fair, prompt, and objective process for resolving grievances.

8. Continuity of Care

  • Receive information about continuing care needs following discharge or transition of services.
  • Be informed of any significant changes to your treatment plan or services.

9. Awareness of Rights

  • Be informed of your rights and responsibilities as a client of Primal Point Health.

10. Disclosure of Relationships

  • Be informed if Primal Point Health has any financial, referral, or business relationships that may impact your care.

Client Responsibilities

As a client of Primal Point Health, you are responsible for:

1. Providing Accurate Information

  • Providing complete and accurate information about your health history, medications, and current condition.

2. Following the Treatment Plan

  • Participating in and following the agreed-upon treatment plan.
  • Informing your provider if you do not understand or are unable to follow treatment recommendations.

3. Respect and Consideration

  • Treating staff, other clients, and clinic property with respect.
  • Following clinic policies, procedures, and program rules.

4. Communication

  • Communicating changes in symptoms, condition, or concerns promptly.
  • Asking questions when clarification is needed regarding care or services.

5. Appointments

  • Keeping scheduled appointments or providing advance notice if cancellation or rescheduling is necessary.

6. Financial Responsibilities

  • Meeting financial obligations related to services in a timely manner, including copayments or fees as applicable.
